Effective Dates: Offer applies to purchases of eligible products made between January 1, 2024 – December 31, 2024.
Limits: Minimum purchase of 30 total cases (any combination of the SKUs listed on the promotional flier). Offer valid only if customer has not purchased any Fisher SKUs in the last 90 days.
Proof of Purchase: Copy of distributor invoices showing purchase(s) of eligible product(s) must accompany form.
Submission Process: Submit one form for all eligible cases purchased during the promotional period. Completed form with proof of purchase must be submitted by February 15, 2025. Please allow six (6) weeks for processing of payment.
